Home with HCBS – When you require assistance to stay in your home as you age, there are many home and community based services to help you for as long as possible.

Caters to individuals who don’t want to worry about having to move from one community to another as they age and their needs change. It offers persons 60 years of age or older a long term continuing care contract that provides for independent living units, residential care/assisted living services, and skilled nursing care, in one location for a resident’s lifetime. Staying connected to community and other individuals to avoid isolation.

Other housing options if you want to live on your own with some basic assistance, but don’t want to have all the chores that go along with having a home. 

If someone who needs high level or round-the-clock care, is unable to live alone with a carepartner (ex. Individual can no longer stay at home due to Alzheimer’s Disease and needs a memory care community), find the NH that best fits the needs of you or your family member to avoid transfers to other nursing homes.Physical, cognitive, and emotional changes that naturally occur, as well as common disabilities.

When independent living is no longer safe, try to anticipate future needs and look for an AL community that can cater to those needs so the individual is not transferred to another care community if the level of care increases.
